Each pre-paid classmate will receive a Bio Book at the reunion. To be included in this book, you must complete the online bio information sheet below these instructions. There is no charge to be included in the Bio Book.

The deadline for returning the bio information sheet is Friday, September 25, 2009 -- two weeks prior to the reunion. We will not accept bio information after this deadline.

If you are not attending the reunion, we still encourage you to join the fun and include your information in the Bio Book. You may purchase the Bio Book after the reunion. Please check back here for details about purchasing additional copies.

• Please do not use all caps when completing this form.
